Streamline Consulting Inc. specializes in providing program management support to safety net providers. Our mission is to collaborate with community health centers, CBO’s and others to build an ecosystem to provide Enhanced Care Management and Community Support services throughout California.

Streamline Consulting is a collaborative team of consultants who specialize in hands-on technical assistance tailored to the needs of your organization and your patient population.
Streamline Consulting staff have led the implementation of a regional Whole Person Care Pilot in California and have work with CBOs, FQHCs, and other vendors to expand Enhanced Care Management and Community Support services throughout the San Joaquin County region. Streamline looks forward to assisting organizations in need of technical assistance to achieve their maximum potential.
